The National Provider Identifier or “NPI” is a 10-digit number that identifies an individual Provider or healthcare entity. USHEALTH Group uses this number solely to verify the business identity and does not share or utilize this information for any other purpose.

This is a one-time registration process. In order to search for your Provider, please have the Provider Last Name or Business Name, Tax ID Number (TIN) or National Provider Identifier (NPI) available.
